Flu Vaccination Forms
Please return the blue Flu vaccination consent form, which have been sent home tonight, by next Friday, 18 October 2019.
Forms need to be returned even if you do not consent to the vaccination for your child.
The flu vaccinations will take place in school, for all years, on Wednesday 13 November.
